MyMedicare Registration at Geriatric Care Australia (GCA)
MyMedicare is a voluntary registration program by the Australian Government that helps formalise the relationship between patients, their general practice, and GP. It enables your GP to access funding to support continuity of care and improved health outcomes - especially for older people receiving aged care.

Why Register for MyMedicare?
By registering with GCA Primary Care under MyMedicare, you help your GP access additional government incentives that directly support services such as:

Coordinated aged care and chronic disease management

Enhanced GP access in residential aged care

Continuity of care from a preferred GP
There is no cost to register, and it allows us to better support you and your health needs over time.
What Am I Agreeing To?
By ticking the consent box on your New Patient Form, you acknowledge that:

You have read this page and understand the purpose of MyMedicare;

If you are receiving GP services from GCA, we may register you under the MyMedicare program using the personal information you provide;

You nominate GCA as your preferred provider of GP care;
-
You understand this registration helps your GP access government incentives to deliver improved services for older people;
-
You are aware that registration is voluntary, and you may withdraw at any time by contacting us or Services Australia.

Who Can Register?
To be eligible for registration with GCA Primary Care:

You must have had at least two face-to-face consultations with a GCA GP in the last 24 months (or one consultation if you live in a rural area – MMM 6–7)

Patients under 18 can be registered by a parent or guardian

Young people aged 14 to 17 must provide informed consent
What Happens With My Information?
When you register, your personal details (e.g. name, Medicare number, date of birth, preferred GP) will be submitted to the MyMedicare system. These details will only be used for:

Processing your registration

Enabling GP incentive payments
-
Coordinating your care between GCA and other services
Your information is protected under the Privacy Act 1988 (Cth) and will only be shared with relevant government agencies, including:

Services Australia
-
The Department of Health and Aged Care

The Australian Digital Health Agency
-
Department of Veterans' Affairs (if applicable)

How Do I Register?
If you're receiving GP care through GCA, we'll take care of the registration process using the information you provide on our New Patient Form. No additional paperwork is needed unless required by Medicare.​ If you prefer, you may also complete a formal MyMedicare Registration Form (PDF) and return it to our staff.

Opting Out or Changing Providers
You can cancel your MyMedicare registration at any time by:

Informing your GP or practice, or
-
Contacting Services Australia

If you register with a different practice in the future, your registration with GCA will be automatically withdrawn.

Terms of MyMedicare Registration
By registering with Geriatric Care Australia (GCA) under MyMedicare, you agree to the following:

Voluntary Participation: You understand that registering in MyMedicare is voluntary.

Preferred Provider: You confirm that GCA is your regular primary healthcare provider.

One Practice at a Time: You understand that you can only be registered with one practice under MyMedicare at a time. Submitting your registration will automatically withdraw any previous registration and notify your previous GP or clinic.

Ongoing Registration: Your registration will continue unless:

You register with another practice,
-
You request your GP/practice or Services Australia to withdraw it, or
-
Your GP or practice decides to withdraw it.

No Cost to Register: There is no cost to register in MyMedicare.

Privacy and Data Handling: You confirm that you have read and understood the MyMedicare Privacy Notice and consent to the collection, use, and disclosure of your personal information by Services Australia, the Department of Health and Aged Care, the Australian Digital Health Agency, and where applicable, the Department of Veterans’ Affairs, as outlined in that notice.

Optional Demographic Info: You may register for MyMedicare even if you choose not to provide optional demographic information (e.g. country of birth, gender, disability status).

MyMedicare Registration Privacy Statement
The law regulates how Services Australia, the Department of Health and Aged Care, the Australian Digital Health Agency and
the Department of Veterans’ Affairs may handle your personal information. Services Australia is collecting your personal infor-
mation to assess your eligibility for MyMedicare and provide services to you and payments linked to your provider as a result of
your MyMedicare registration. Your information will only be shared with relevant government agencies such as the Department
of Health and Aged Care, Australian Digital Health Agency and the Department of Veterans’ Affairs, where you have agreed, or
where the law allows or requires it. The MyMedicare Privacy Notice describes how your information will be managed consistent
